Ekland said the actress is unable to show a range of emotions because her face has been frozen by the anti-wrinkle treatment.
Kidman, who recently gave birth to daughter Sunday Rose, has never admitted to using Botox.
Ekland, 65, said: "It's fatal when actresses use Botox. I remember seeing Cold Mountain, and it really looked to me like Nicole Kidman had been using it.
"Her face was neither sad nor glad - nor anything, she was just like a painted doll. I thought: 'Why would she do that?'"
A Botox expert recently accused Kidman of looking like a bat because of her alleged overuse of the treatment.
