The image shows X-Man Colossus wearing the helmet of the team's long-time foe, the Juggernaut.
The tie-in with the summer event sees Piotr Rasputin make a deal with the demon Cyttorak to acquire Cain Marko's former power after he acquires one of the hammers of the Worthy and is transformed into Kurrth, the Breaker of Stone.
"Colossus is a character who's always leaned towards sacrificial," writer Kieron Gillen told Marvel.com. "He's the sort of person who'll give up something for someone else, no matter what the cost. And this is that abstractly good character leading him into a very bad place.
"Traditionally, The Juggernaut has been written primarily like any other super-powered character origin. He finds a magic gem and becomes The Juggernaut! Except Cain, personality-wise, has generally been in line with Cyttorak.
"Piotr isn't that guy. The reference point I've put in the scripts for the artist isn't anything from the traditional superhero canon, it's The Exorcist. This is a deal with a devil and trying to live with it."
The image follows on from a previous 'MMXI' teaser showing Colossus donning the distinctive domed helmet.
Fear Itself #4 was teased last week after the release of issue #3.
Uncanny X-Men #543 will be released in September.
