Full Summary: Sam, Dean and John were left for dead after the Demon plowed a semi into the Impala. Now, one of the Winchesters hovers between life and death as the surviving family must face an old and powerful adversary.

In the aftermath of the cliffhanger crash, the 'driver' gets out of his lorry cab and walks toward the boys' car. Sam wakes and threatens to kill the demon which possessed the driver. Before Sam has chance to pull the trigger of the colt, the demon escapes from the driver's body.

At the hospital Dean gets out of bed and is puzzled as to why there's nobody around in the hospital. He walks up to the reception desk to ask where he is but everybody seems to be ignoring him. He rushes around the wards and is stunned when he walks into a room where he's laying…

Sam walks into the same room, devastated by his brother's condition. Dean can't understand why Sam can't hear him. A doctor tells Sam that John, their father, is awake. The doctor also explains that Dean suffered series injuries to his liver and kidney as well as to his head. They won't know Dean's full condition until – and if – he wakes.

Sam talks with his father at his bedside where John enquires as to the whereabouts of the colt.

At the scrap yard, Sam surveys the wreckage of Dean's classic car. Sam hands Bobby a list of things which his father requires for protection from the demon. Bobby, however, has something important to tell Sam…

Back in the hospital, Dean watches as his father sits next to his comatose body. Dean can't understand why his father hasn't called anyone, hasn't done anything. In the corridor, Dean spots a spirit travel past. He follows it to another room in which a woman dies.

Sam returns to the hospital and confronts John about the products he requested from Bobby: they aren't for warding off demons, they're for summoning them. John informs Sam that what he's doing is for Dean. Dean smashes a glass on the floor – something which both Sam and John see.

In Dean's hospital room Dean's lapsing. He sees the evil demon looming over his body. He grabs for it and it disappears - he's still alive.

Elsewhere in the hospital, Dean hears another girl, Tessa, shouting to see if anyone can see her. Dean takes Tessa to see her body in her hospital room. She doesn't understand what's happening.

Sam talks to his father about the possibility of Dean's spirit being present at the hospital. Another emergency elsewhere in the hospital results in another loss of life.

Sam brings back a ouija board in the hope of contacting Dean. After a beyond-the-grave discussion, Sam decides to tell his father that Sam's OK. But John's not in his bed…

Down in the basement, John prepares to summon his demon while, upstairs, Sam flicks through his father's diary. Dean catches a glimpse of one of the pages and realises he's been taken for a fool.

Dean confronts Tessa about reapers. They can alter human perception and portray themselves as pretty young women – much like Tessa. She informs him that it's his time to go; he's living on borrowed time already.

Meanwhile, in the basement, John chants in the hope of summoning the demon. Eventually, he's not fooled by the demon's guise and threatens it with the colt. John reveals that he wants to make a deal.

The demon accepts John deal: the colt and bullet in return for his son's life. The Demon asks John if he knows the truth about Sam and the 'other children'. John admits he's known for a while and acknowledges Sam doesn't. But the Demon still isn't happy with the deal – he wants something else.

The demon executes his part of the deal and takes over the body of the reaper who, in turn, brings Dean back to life. The reunited brothers have a heart-to-heart with each other and their father. John reminisces about the earlier years and realises how thankful he is to Dean. John asks Dean to watch over Sam and whispers something in his ear and walks out of the room…
