The downloadable episode will be available via Xbox Live Arcade for 1600 Microsoft Points (£12).
Players will take control of nightclub assistant Luis Lopez and can expect to "struggle with the competing loyalties of family and friends, and with the uncertainty about who is real and who is fake in a world in which everyone has a price", according to a synopsis.
The add-on follows in the wake of previous episode The Lost And The Damned, which was also released as an Xbox 360 exclusive in February.
A standalone disk titled Episodes From Liberty City, comprising both episodes, has also been confirmed for release. A copy of GTA IV will not be required to run the game.
The Ballad Of Gay Tony will arrive on Microsoft's online storefront on October 29.
