The sequel was announced with a cinematic trailer showing a knight travelling across a range of new dark fantasy environments.
Publisher Namco Bandai confirmed that it will feature a new hero, story and game world to explore.
Dark Souls 2 is teased to feature a "distinct" and "revamped" twist on the game's acclaimed multiplayer experience that sees players leave messages to one another in the game world.
"This new chapter in the Dark Souls saga presents opportunities for us to drive innovation in gameplay design, develop an entirely new story, and expand the scope of the world in which the player interacts with the game," said director Tomohiro Shibuya.
"We have taken these necessary steps with Dark Souls II in order to evolve the overall experience of the Dark Souls series.
"The entire development team is striving to make Dark Souls II an experience that is fresh while not forsaking its roots in presenting players with challenging gameplay."
He added: "Our goal is to surprise and delight our fans with new experiences and plot twists while enticing new players to join our dark journey."

Namco Bandai added that more details will be revealed in the months leading up to release.
Dark Souls 2 is in development by From Software for Xbox 360, PlayStation 3 and PC, and has yet to be given a release date.
The original was released on Xbox 360 and PlayStation 3 in October 2011 with 1.5 million units shipped worldwide.
Earlier this year, the PC port Dark Souls: Prepare to Die Edition released including the new Artorias of the Abyss content.
