The wide-ranging overhaul will also see LBC News 1152 presenters James Hartigan and Petrie Hosken moving to FM to host drivetime from 4pm to 7pm.
LBC 97.3 FM programme director Jonathan Richards said: "Jeni has made a fantastic impression on everyone at LBC, both on and off the air. Her weekend show is an excellent listen and I know her infectious personality will make her new weekday afternoon programme unmissable.
"James and Petrie will bring Londoners a fast-moving round-up of the day's news whilst inviting listeners to comment on what's been happening, as we continue 'London's biggest conversation'."
Former afternoon and drivetime hosts Alison Bell and Paul Ross will remain with the station and make "occasional appearances".
