Downey Jr, who took on the title role in the Guy Ritchie-directed film, was joined by co-star Jude Law and celebrities including Heather Graham and Gwen Stefani on the red carpet in Leicester Square.
According to PA, organisers had brought in old-fashioned street lamps to help recreate Victorian London for the event.
Downey Jr spoke to reporters about his on-set chemistry with Law, who plays Holmes's sidekick Dr John Watson in the movie.
He said: "They're talking about Jude and I like we should be doing romantic comedies together or something. But this film is not a comedy and it's a love affair of sorts.
"But I think that Holmes and Watson are aspects of all of us and I think that we knew when to yin and yang back and forth and we were just a good team."
Meanwhile, Ritchie claimed that he could not think of "anyone more appropriate" to play Holmes than Downey Jr.
Sherlock Holmes will go on general release on Boxing Day in the UK.
