The track, titled 'Collide', will be released in the UK on September 4. The artwork also confirms that Ministry Of Sound DJ Avicii will be listed as a feature artist on the record.
The cover sees the X Factor champion dressed in a '60s-style maxi dress.
'Collide' suffered a major plagiarism claim upon its debut last month, with Ministry Of Sound and Avicii insisting that they had not given permission to use the song's instrumental for the single.
However, the artists came to an agreement earlier this week, which will see the track receive a release under both of their names.
'Collide' will serve as the lead single for Lewis's upcoming third studio album, which will be released this November.
Lewis, 26, is reportedly working with grime producer Naughty Boy on material for the record.
Leona Lewis has sold over nine million records worldwide since she won The X Factor in 2006.
