The punk icons will their 'Back To Front' performance to the Manchester O2 Academy on May 25 and London's O2 Academy Brixton the following day.
"They'll kick off with new and contemporary material featuring the current line-up of Pete Shelley (vocals / guitar), Steve Diggle (vocals / guitar), Chris Remington (bass guitar) and Danny Farrant (drums)," organisers confirmed.
"Part two sees Pete and Steve perform the United Artists 'classic years' tunes featuring the classic line-up of Shelley / Diggle and Steve Garvey (bass guitar) and John Maher (drums).
They added: "The evening's proceedings will be brought to a clamouring close in part three by Pete, Steve, and John being joined on stage for the first time in over 33 years by original co-founder and frontman, Howard Devoto, to close the evening by performing the four tracks featured on the band's debut Spiral Scratch EP."
The gig will not feature bassist Tony Barber or drummer Phillip Barker, who played on five Buzzcocks studio albums from 1993's Trade Test Transmissions to 2006's Flat-Pack Philosophy.

Devoto, real name Howard Trafford, and Shelley, real name Peter McNeish, founded Buzzcocks in 1976. Their self-released Spiral Scratch EP is credited with kickstarting the DIY movement in the UK.
The duo played a key role in the early days of punk when they invited Sex Pistols to play two shows at Manchester's Lesser Free Trade Hall in June and July of that year, making their own live debut at the second gig.
Devoto left the band in early 1977 to form Magazine, who themselves reformed in 2009. Shelley became the group's singer and Diggle moved from bass to guitar.
Buzzcocks split in 1981 but reformed in 1989 and have gigged and released new albums since - their reformed incarnation now being active for 17 years longer than their punk-era versions. The band famously supported Nirvana on their final European tour dates in 1994.
In 2002, Shelley and Devoto reunited for the one-off album Buzzkunst under the name ShelleyDevoto.
