The heavy metal icons play the O2 Academy Birmingham on Saturday, May 19.
Tickets go on sale this Friday, May 4 at 9am and are priced at £45. Proceeds from the show will be donated to Help for Heroes.
The date is the band's first in their native Birmingham for 13 years and precedes their headline slot at this year's Download and Lollapalooza festivals.
Black Sabbath were forced to cancel a number of dates on the European leg of their world tour after guitarist Tony Iommi was diagnosed with lymphoma.
The group's original drummer Bill Ward is believed to have pulled out of the reunion due to a contractual dispute with his bandmates Ozzy Osbourne, Iommi and Geezer Butler.
