The singer-songwriter was announced as the special guest for the event, completing the bill for the 2012 festival.
"To say seeing Patti Smith at the festival is exciting is a bit like saying the Dalai Lama is quite religious," the festival directors quipped on the official End of the Road website.
"There's truly no other musician like her working today."
End of the Road Festival takes place from August 31 to September 2 at the Larmer Tree Gardens, North Dorset. Adult tickets are priced at £150.
This year's bill includes Grizzly Bear, Grandaddy, Midlake, Beach House, Tindersticks, Van Dyke Parks, Graham Coxon, Anna Calvi, Robyn Hitchcock and others.

She was joined at last weekend's Hop Farm Festival by regular live collaborator Patrick Wolf.
"On the way to Hop Farm festival to play some fiddle with Patti + her band, what a birthday surprise! <3 + thanks for your loving wishes xPW," Wolf said on Twitter.
Smith last month released her 11th studio album Banga.
The record features a song in memory of late singer Amy Winehouse called 'This Is the Girl'. It also includes 'Nine', written for Smith's friend Johnny Depp.
