The cover features the rapper holding a picture of himself as a child up to a crowd he is performing for.
Example revealed on Twitter that he took the photograph himself during his V Festival set in Stafford earlier this year.
The artwork is designed by the man behind album covers for The Prodigy and The Streets.
"The guy who designed my new album cover also designed the covers for Fat Of The Land and Original Pirate Material," the 'Kickstarts' star tweeted.
Example will release his fourth studio album The Evolution of Man on November 18, preceded by lead single 'Say Nothing' on September 16.
He has also announced plans to embark on a headline UK tour next year.
