The awards show, held to recognise jam bands and acts associated with live, improvised music, will be held on May 7 at New York's Madison Square Garden.
Phish formed in 1983 and quickly became renowned for their extended jazz-infused jam sessions. The group performed together for over 20 years before splitting in 2004. However, guitarist Trey Anastasio said a reunion is "definitely not 100% out of the question".
Previous recipients of the Jammy's Lifetime Achievement Award include B.B. King, The Grateful Dead and Frank Zappa.
