The bash, organised by the Welsh Rugby Union, takes place at Cardiff's Millennium Stadium on Monday, May 26.
Players including captain Ryan Jones and player of the tournament Shane Williams will attend the celebrations.
"This is the opportunity for the supporters to salute the players and coaches who delivered so many victories for Wales this past season," WRU chief executive Roger Lewis told the BBC.
"Next Bank Holiday Monday will be a day for rugby lovers, music lovers and anyone who is proud to be Welsh or passionate about sport in Wales."
