Speaking at Wednesday's premiere of Knightley's latest film The Duchess, Palmer-Tomkinson said she can relate to the actress because she has a similarly slender frame.
On the subject of media jibes, she said: "You just never read newspapers - that's what I do, I haven't read one for about 20 years. Look, we are the way we are - some people are bigger and smaller.
"And listen, if you've got a God-given face like that [in Keira's case]... I mean, I think people are always going to be personal if you're in the public eye and it doesn't look like she is somebody who lets it bother her."
The socialite, who will host Living reality series The Underdog Show in the autumn, continued: "Being skinny has always been a massive sort of aspiration and a thing. I mean, personally I think it's just as bad to go up to someone and say you're way too thin as going up and saying you're way too fat.
"I certainly wouldn't dream of ever being so personal to somebody."
