The actor allegedly spent 20 minutes browsing the Green House store in Venice, before spending £150 on "one of the biggest bongs" and a 'monkey' pipe.
According to Life & Style magazine, Wilson was "friendly" but feared being seen buying the items, which are used for smoking marijuana and other substances.
A source said: "He was worried that someone might have seen him coming out. He hid behind a FedEx truck for 30 seconds before darting to his car."
Friends now fear Wilson is in danger of a drug relapse following his attempted suicide last August.
