The chiffon Elizabeth Emanuel garment, which appeared in Vogue, is expected to fetch up to £25,000 in the Atrium sale.
A miniature copy of Diana's wedding dress made from left-over material from the original will also go under the hammer in the ten-day sale.
Emanuel told ITN: "It was through this blouse that our relationship with Diana started, and it was always a tremendous honour and privilege to work so closely with her."
A total of 200 of the designer's pieces are being sold, including dresses worn by Jane Seymour and Elizabeth Hurley.
