Swiss officials say that they have been asked to deliver the director on sex charges relating to an incident in 1977, the BBC reports.
The Swiss justice ministry said that Polanski may appeal the decision if it approves the request from the US authorities.
A statement read: "The US Embassy in Bern submitted the formal extradition request... within the deadline of 40 days stipulated under the bilateral extradition treaty."
Polanski fled on the eve of sentencing in 1978 after pleading guilty to unlawful sex with the then 13-year-old Samantha Geimer.
The Chinatown director was arrested last month in Switzerland and recently lost an appeal to be released on bail.
