The Diff'rent Strokes actor, who was hospitalised last month after suffering an intracranial haemorrhage, died after his ex-wife Shannon Price gave permission for the machine that was keeping him alive to be switched off.
In Instructions for Health Care paperwork obtained by TMZ, Coleman ticked the box titled Choice to Prolong Life, which read: "I want my life to be prolonged as long as possible within the limits of generally accepted health care standards."
Coleman's agent Robert Malcolm has previously claimed that friends and family of the star begged Price not to "pull the plug" but she ignored their requests.
Price has said that she made the decision based on the advice of doctors.
