The comedian was on the NBC show to promote his new greatest hits collection I Did My Best when he revealed that he was a "big fan" of fellow guest Swift, branding her "America's sweetheart".
"I thought I would do something special and a memorable moment for you tonight," he told her, before stripping off his shirt to reveal a picture of Swift's face etched onto his chest.
"This is dangerously close to stalker territory!" joked Leno, as Cook reached for a guitar. "It's great," responded Swift. "Awesome!"
Cook then sang an original song for Swift - which included a promotion for his new project - with the lyrics: "Taylor Swift / You're a gift / Give me a lift to your heart / Beating inside my chest / I did my best / Download it now / Now back to you Taylor Swift / Stand up girl / Take a bow / Show us that smile / Don't be shy / I hope her fans encourage her now."
Swift, who was promoting her NBC Thanksgiving special, then stood up to curtsy for the audience as Cook encouraged them to sing along with him: "We love you Taylor, Taylor, Taylor Swift."
I Did My Best is available now. The Taylor Swift: Speak Now special airs on November 25 at 8pm ET on NBC. The Tonight Show With Jay Leno airs weeknights at 11.35/10.35c on NBC.
