Sheen admitted on Monday that his sobriety could "unravel" very quickly.
During an appearance on HLN, the Celebrity Rehab physician explained that he doesn't believe Sheen is taking his recovery seriously.
"He's becoming the poster child for distorted thinking and denial," he said. "It's very sad."
Pinsky also cautioned that if Sheen does not begin to make his sobriety a priority, he will face dire consequences.
"It's not about telling somebody how to live their life or the kind of life they need to lead," he explained. "Given the condition he has and the behaviors he is manifesting, [and] the substances that he's relating to - it's very predictable where this going to go. It's going to go to horrible places."
Earlier this week, Sheen reportedly tested negative for drug use.
