The actress was ordered to attend a residential rehabilitation facility for six months and given five years of informal probation.
Warren was arrested back in May for crashing her vehicle into three parked cars as well as allegedly kicking the arresting officer.
After being taken to a nearby police station, she managed to escape her handcuffs and flee the station before being apprehended once again only minutes later.
"I take my actions and their consequences very seriously," a representative for the actress told People in a statement. "I can't express enough how grateful I am that no one was injured or hurt.
"Moving forward I am focusing all my efforts on my career, my family, and being a good influence on others."
Warren is best known for starring opposite Mark Wahlberg in the Tim Burton-directed 2001 sci-fi Planet of the Apes.
She will have to return to court on October 7 to discover whether she has been ordered to pay any restitution.
