Dr Giovanni 'John' Palumbo violated a court order to stay 500 feet away from Twain at all times by attending the Juno awards in March.
Palumbo was arrested at the ceremony when he was recognised by a member of Twain's entourage.
On Thursday, Palumbo appeared in a Toronto court to answer three charges of failure to comply with a court order and one count of criminal harassment.
Twain testified via satellite that Palumbo sent her disturbing letters and visited her brother-in-law's auto repair shop in order to learn more about her.
"The manipulation scared me a bit," Twain said during her testimony.
The hearing came to an end soon after an agitated Palumbo rose to his feet and issued a statement, in which he referred to Twain by her real name Eilleen Edwards.
"Eilleen, you can trust me, I'm going to plead guilty… I've been offended too much, your honour," he said.
Palumbo's trial is expected to continue today, reports The AP.
