The model is also launching a £250,000 lawsuit against her former manager Claire Powell, management company Can Associates, Andre and former friend Jamelah Asmar.
Price filed papers at the High Court last week, and is seeking damages for "misuse of private information", which left her with "serious distress", reports The Mirror.
Powell has been accused of requesting false information about the model after they parted ways with Price's employee Nicola Partridge.
The e-mail in reply from Partridge had alleged that Price was on the "point of having an affair" with riding instructor Andrew Gould.
Private medical information was also reportedly in the e-mail, which Price has said is a breach of contract.
The court papers also include a statement said to be written by Asmar, which was reportedly put together by Andre's lawyers in order to create a negative news story about Price.
The statement claims that Price had a "clandestine romance" with Gould and may have aborted a child with him.
The affidavit is also said to include the name of a man who allegedly raped the model "some years ago".
Katie Price and Peter Andre were married for four years and have two children together. They have been involved in several legal battles in recent years.
Last year, Price paid Andre an undisclosed sum and later apologised over claims that he cheated on her.
The current lawsuit was first rumoured to have been launched by Price back in March, but Claire Powell has now confirmed the new legal proceedings from her former colleague.
She said: "We consider that the claim is without merit. The matter is in the hands of our lawyers and will be defended vigorously."
Katie Price recently said that her marriage to Peter Andre ended because he wrongly thought that she had an affair.
