The model pleaded guilty to a charge of reckless assault - saying she threw the phone but did not mean it to hit anyone - after agreeing a plea bargain with prosecutors.
She was also ordered to complete a two-day anger management course and pay medical bills of $363.33 (£184.90) to Ana Scolavino, the injured maid, who needed four stitches.
Campbell told Manhattan Criminal Court: "During the morning of March 30 2006, I threw a cellphone in the apartment. The cellphone hit Ana. That was an accident, because I did not intend to hit her. I am advised that Ana was hurt, and I am deeply sorry about that."
David Breitbart, her lawyer, requested that the community service be something completed inside, citing her safety as a reason.
Campbell has earned a reputation for aggressive behaviour after facing a list of accusations of violence, mostly from employees.
