He is also being investigated for brandishing a deadly weapon and attempting to influence a witness in a domestic violence case.
The incident took place at the Intercontinental Hotel on April 30. Police said that Rodman "hit a woman and she suffered injuries to her arms".
"His girlfriend of four years sustained visible physical injuries," said Los Angeles city attorney spokesperson Nick Velasquez.
Following his arrest last month Rodman checked into a Florida rehabilitation centre to deal with his alcohol problems.
Rodman will appear in court on May 22.
