Elisabeth Sladen - The Autobiography will be available for purchase on November 7 in the UK and December 15 in the US. The autobiography will feature the story of the actress's post-World War II childhood through to her starring role on The Sarah Jane Adventures.
Publisher Aurum Press touted Sladen's book as "funny, ridiculous, insightful" and "an entertaining and a fitting tribute to a woman who will be sadly missed by millions".
Former Doctor Who star David Tennant has written a foreword for the book.
Sladen first appeared as Doctor Who companion Sarah Jane Smith in 1973 and worked with both Jon Pertwee and Tom Baker.
During her tenure on Doctor Who, Sladen starred in many well-loved serials, including 'Genesis of the Daleks' and her final adventure 'The Hand of Fear'.
Sladen reprised the role of Sarah Jane Smith to celebrate Doctor Who's 20th anniversary in 'The Five Doctors' and returned in 2006 when the series was revived by Russell T Davies.
Her return to Doctor Who proved so popular that she was granted her own spinoff series The Sarah Jane Adventures.
Sladen's death from cancer in April drew tributes from Davies and current Doctor Who showrunner Steven Moffat.
Matt Smith said that he will remember Sladen most for her "grace", while Tennant commented that he had been "enchanted" by Sladen from an early age.
