Rob's surprised to see how settled Karen is at The Mill, and realises she's found herself a challenge and is obviously valued and loved by her new colleagues. Karen insists that Jimmi check Rob over.
Rob, who vaguely knows Jimmi through work, dislikes him for being suave, and sympathetic to prisoners. He's further disgruntled when Jimmi insists on at least 24 hours of bed rest. When Rob and Karen return to their new bedroom, it's lost all of its sparkle. Karen is hurt and embarrassed that Rob didn't make more effort with her new friends.
When Zara sees Ruth having a quiet moment, she passes her back the lost property clothes, all freshly dry cleaned. Ruth enjoys the fledgling bond between them.
A little later, Zara has a patient Mrs Bissett who is experiencing the menopause. She is well into her 50s but is finding it very hard to accept that the bloom of her youth is fading. Through clenched teeth, Zara urges the woman to look on the bright side – she has several children, hasn't lost her sex drive, and her symptoms are relatively minor.
The woman still complains and says its hard for a slip of a thing like Zara to understand, but the menopause puts a full stop on a woman's life. Zara passionately argues this isn't the case, but the patient is determined to be miserable about it, pushing Zara over the edge. Ruth hears the raised voices and comes in.
Zara stalks out and Ruth talks to the patient who guesses this is close to home for Zara and is sympathetic. Ruth neither confirms or denies it, but suggests she sees another Doctor instead. After dispatching the woman, Ruth goes to find Zara to sympathise and help her open up, but Zara's having none of it.
Meanwhile, Bryoni (22) is the latest in a family line of mediums, and learnt her calling from her mother who she idolises. However Bryoni's mother died when she was ten, and her care passed to her aunt Cynthia (54), who now takes care of the business side of things.
Bryoni is exhausted and suffering pain in her wrist which Simon has diagnosed as rheumatoid arthritis, but under the guide of her aunt, she is refusing treatment, preferring to trust in fate, as she's told her mother used to do.
Unable to accept Bryoni doing herself such harm, Simon digs around, and discovers Bryoni's mother accepted treatment in hospital after she gave birth to Bryoni. Cynthia had been hiding this, worried that her niece being in hospital would be bad for business.
Finding this out, Bryoni feels empowered, and she decides she will get treatment for her RA, take a holiday, and then go part time – it's time she had a life. Having made the decision Bryoni feels the presence of her mother.
