Karen starts to accuse her of self prescribing and threatens to tell the GMC. Zara gives in and admits she has been injecting herself with testosterone in the hope that it will delay her early menopause. Karen is shocked and then becomes sympathetic; has Zara frozen her eggs? Does she want children?
Zara cringes but we see a part of her appreciates the consideration. Karen finally leaves and gives Zara a hug, much to her horror, and assures her everything will be fine as long as she never, ever, touches her son again. If she does… she'll break her arms.
Ocean asks Julia to come with her, Marlin and Liam to Somerset to complete an attachment parenting course. Julia's so pleased she tells Heston to deal with the Zara/Karen scenario on his own. However later Julia can't help but criticise Ocean's decision not to give Marlin the MMR jab. The two argue again and Ocean retracts her invitation and leaves telling Julia she hopes she's happy alone in her world where she's always right.
Meanwhile, dodgy dealer Billy (20's) bumps into Lily on his way out of the police station having spent the night in the cells. Lily and he banter, clearly knowing each other well.
Back at home, Billy hears his neighbours arguing and their daughter Shannon (8) crying. He tries to block out the noise but Shannon appears in his house, still crying. He tells her she can't keep coming around and Shannon, upset, throws a DVD at him. She brought it for him as a present.
Alone and feeling bad Billy watches the DVD and is shocked to find its hard porn. He goes back to the police station to ask WPC Ottosen to help Shannon but she distrusts Billy and sends him on his way. Lily overhears and gives Billy a chance. In her office he explains about how Shannon is beaten and subjected to blue movies.
Lily agrees someone should look into it so goes back to Billy's house with him, much to Ottosen's annoyance. Lily sees how Shannon's drunk and abusive parents treat her and phones social services.
As the police drive her parents away, Shannon runs up to Billy, excited as the social worker said she could have pizza for dinner tonight. Emotional, Billy asks why Lily listened, to which she quips that sooner or later he had to be telling the truth.
