S03E16: 'One Of Us'
On their way back to the camp, Sayid reveals his suspicions about Juliet’s intent and wants to interrogate her, but Jack steps in and protects her. When they reach the beach, Sawyer comes over and expresses his anger that one of ‘The Others’ has joined them. Hurley is given the mission of keeping tabs on the new arrival, revealing to her the grim fate that Ethan Rom experienced.

In a flashback, we’re taken to Juliet’s pre-island life. She has signed up for the Dharma Initiative and bids farewell to her preganant sister, promising to return soon. Ethan and Richard are on hand to escort her, checking that she’s maintained her silence and administering a tranquiliser on her for the journey. When she awakens, she finds herself on a submarine and emerges to find herself at the island alongside Ethan.

We next see Juliet in an operating theatre witnessing a woman die during childbirth. She later asks Ben if she can return home to see her sister, but he replies that her cancer has returned and she will be dead. However, Ben says that a figure named Jacob will cure this cancer as long as Juliet stays.

Back in the present, pregnant Claire takes a turn for the worse and appears critically ill. Juliet reveals that this has been caused by medication given to her when Ethan abducted her, with this line of action being taken to prevent her dying. However, she explains that there must be a delayed reaction to the medication on her bloodstream. Juliet claims to have designed the drug, having been brought to the island to solve the mystery as to why women can’t have children there. Juliet then asks to go to a location nearby where Ethan had stored some serum that can help keep Claire alive. Sawyer and Sayid secretly follow her, then aggressively confront her. Juliet then hits back by detailing incidents in both mens’ past that show how they have no moral leg to stand on. They let her go and give the serum to Claire, who soon recovers.

Via flashback, we see the moment when Juliet discovered that Ben has a tumour in his back. She now believes that he lied about his sister being cured of cancer earlier, as he can’t cure his own affliction. However, Ben insists he was correct. Next, we move forward to the day that Flight 815 hit the island. At one of the stations, Mikhail is instructed by Ben to find detailed information on the passengers. Juliet is also shown a video feed from America featuring her sister – still alive – with hertwo year old child. A newspaper with the day’s date shows that the footage is current.

Juliet is subsequently seen with Ben discussing their plans before The Others vacate their camp. We learn that Ben instructed her to handcuff herself to Kate and claim she was left behind and gassed. It also emerges that Ben has manufactured Claire’s illness via an implant. Juliet’s curing of Claire is manufactured to bring the rest of the beach camp survivors onto her side and gain their trust.
