The next day, Dean speaks to Lisa on the phone, explaining that he is not far away and will visit her soon. Sam informs his brother that six girls, all around the same age, have gone missing in seven days. A seventh victim - Kristen - has just joined the ranks and the brothers agree to visit her home. They investigate Kristen's bedroom and discover that she has a popular fetish - her room is covered with vampire-themed posters and merchandise. Sam hacks into her laptop and discovers that her e-mail inbox is full of messages from a man claiming to be a vampire. The brothers speculate that real vampires could be using their new romantic image to lure in willing victims. They discover that Robert and Kristen agreed to meet at the Black Rose and decide to pay the bar a visit.
That night, a blood donor van is hijacked by a gang of vampires. Samuel calls Sam to explain that a pattern is emerging - girls go missing, then a donor van is attacked. Sam is curious as to why the vampires are stealing blood bags when they have the girls as willing victims. He and Dean visit the Black Rose and notice two men in dark clothing flirting with a pair of girls. The couples part ways and Sam and Dean split up to follow both potential vampires. Sam follows his vampire through the club and into a storage room in the back. The creature attacks him, but Sam is able to decapitate it with a machete.
Outside the club, Dean saves the girl from her potential vampire, only to discover that he is a human wearing glitter and fake fangs, cashing in on the vampire craze "to get laid". Dean releases the pretender, but is soon confronted by a real vampire - Boris. He overcomes Dean, but Sam arrives just in time. However, rather than save his brother, Sam watches with a smile as Boris infects Dean with his vampire blood. Sam rushes towards Boris, who climbs up a building to escape.
Later, Dean struggles with his new enhanced senses. Dean tells Sam that when Samuel arrives, he is going to ask his grandfather to kill him. Dean asks Sam why he isn't freaking out - he can hear his heartbeat, and it's steady. Sam claims that he is trying to remain calm. Dean enters the bathroom and stares at his reflection in the mirror, noticing a new pair of fangs. Sam attempts to check on his brother, but Dean has escaped through the bathroom window.
Dean appears in Lisa's bedroom and she wakes with a start. He tells Lisa that he is thankful for their time together, but she realises that something is wrong and begs Dean to confide in her. Dean struggles to contain his new blood-lust and rushes out of Lisa's bedroom. He finds Ben in the hall, but pushes the boy out of the way in desperation and flees the house.
Samuel unites with Sam and chastises him for losing track of his brother. However, Dean returns, explaining that he did not feed on any humans during his absence. He orders Samuel to kill him, but he explains that he can cure Dean instead. Samuel reveals that one of his ancestors once developed a cure for the vampire condition, but that it requires the blood of the vampire that turned the victim. Now able to detect the vampire nest by smell, Dean decides to hunt Boris down. Once Dean has departed, Samuel accuses Sam of allowing his brother to be turned, revealing that he informed Sam of the cure months ago. Sam denies that the conversation took place, but Samuel speculates that Sam wanted to use Dean as an inside man to infiltrate the vampire nest and capture the alpha vampire. However, Sam angrily denies the accusation and storms out.
With Boris distracted, Dean attempts to attack him, but the vampire fends him off. However, the entire vampire nest - including Dean - receives a psychic message from the alpha vampire, which causes them to pass out. In his unconscious state, Dean suffers from bizarre visions involving a pair of young vampire girls and a sinister man with sharp claws. Dean wakes and attempts to escape from the nest, but is confronted by Boris and his vampire horde. He uses a machete to decapitate a large number of the creatures, including Robert. However, Boris tells him, "You've stopped nothing, you know that? This is much bigger than you and me."
Sam and Samuel arrive at the nest to find Dean standing over the decapitated corpse of Boris. Later, Samuel prepares the cure and Dean quickly drinks it down. Dean begins to vomit a thick black liquid and, as he recovers, his mind flashes back through recent events. He relives the moment he was turned and has a vision of Sam, smiling. With Dean returned to his normal human state, Sam questions him about what he discovered within the nest. Dean explains that the vampires were receiving psychic messages from their alpha, ordering them to begin a vampire "recruitment drive". He also learnt another terrifying fact - vampires no longer fear hunters.
As Sam and Dean prepare to leave, Dean tries to call Lisa, but only reaches her answer-phone. Sam tells him that he is sorry that his relationship with Lisa did not work out, but Dean tells his brother that at least he can always rely on his help and support. However, a wary Dean still seems suspicious of Sam as they drive away in the Impala.
