Sam finds his room at the hotel, discovering a number of newspaper clippings inside which relate to violent incidents and murders. Robin is also perturbed when she finds multiple IDs with different names, all with Sam's picture. Sam spots a news article reporting Eleanor's Visyak's disappearance and recalls tracking her down with Dean and Bobby. The trio discover the badly wounded Eleanor, who has been tortured by Crowley and Castiel. She reveals that the pair need both virgin blood and the blood of a Purgatory native to open the gate, and now they have plenty of both. Eleanor explains that the ritual will coincide with an eclipse the following day, but dies before she can reveal Castiel and Crowley's location.
At that moment, a frustrated Castiel appears and attempts to reason with the Winchesters one last time. When they refuse, Cas apologises for what he is about do and touches Sam's mind. Sam then wakes up back in the hotel room with Robin. Recalling Bobby's name, Sam finds his ally's address in South Dakota. He discovers a set of keys and decides to go find his friends, but Robin is worried and warns Sam that he may not like what he finds. Suddenly, Sam hears a noise and knocks Robin to the ground, as a bullet whizzes through the air. A shocked Sam looks up and spots the shooter - his exact double. It is revealed that Sam is actually comatose - his experiences with Robin are all taking place within his mind.

The Soulless-Sam appears to corner his foe and shoot him dead, but Sam has in fact created a dummy using his jacket and shoots his double. As the soulless Sam dies, he warns Sam that he still has to confront an even worse side of himself. Sam absorbs the energy of his counterpart and his memories of the past year are returned. He then returns to Robin, now fully aware of her true identity. In another flashback, it is revealed that Robin was held hostage by a demon and that Sam himself shot her dead to remove his enemy's leverage. Her true purpose revealed, Robin vanishes.
Meanwhile, Balthazar visits Dean and Bobby and brings them Cas and Crowley's location, but refuses to help further for fear of incurring Castiel's wrath. As they prepare to leave, Dean leaves the comatose Sam a note with the address of Cas and Crowley's hide-out. At that very location, Crowley presents Cas with a jar containing the mixture of virgin blood and Eleanor's blood. Castiel than betrays Crowley, declaring that he shall receive none of the souls of Purgatory. Though the King of Hell is furious, Cas presents him with two options - flee or die.

Dean and Bobby observe Cas's hideout from outside, but are forced to take cover inside the Impala as a huge plume of black demon smoke descends on them. Crowley confronts Cas and reveals that he is now immune to his powers, as he has struck a new deal - with Raphael. Seemingly defeated, Castiel hands over the blood jar to Crowley and Raphael and departs. The villains prepare to open the portal to Purgatory, but Dean and Bobby recover from their attack and enter the hide-out. Dean hurls the angel blade at Raphael, but he is able to catch it, and Crowley uses his power to incapacitate both Bobby and Dean. Meanwhile, Sam closes in on the hide-out, but his memories of Hell are beginning to break through.

Castiel, now powered by millions of souls, has become distant and full of ego. Dean insists that he must get rid of his new power, but Cas rejects Dean's friendship and insists that he will use the souls to punish the followers of Raphael. At that moment, an unstable Sam arrives and stabs Cas with the angel blade, but the weapon has no effect. Castiel decrees that he is no longer an angel, but the new God. He tells Bobby and the Winchesters: "You will bow down and profess your love unto me, your Lord, or I shall destroy you."
